How to Apply Stage 1: Brief Proposal
Please follow these instructions carefully only if you wish to submit an unsolicited brief proposal directly to ELF.
You can submit your brief proposal for your unsolicited project online, by mail or by fax. If you apply by mail or fax, your brief proposal should be sent on your institution's letterhead, and should not exceed five typed pages. You should summarize your project by responding to each point below with a short paragraph:
- Briefly state the problem.
- Describe the project.
- Describe the project deliverables.
- Indicate what constitutes success and how you will measure it.
- Explain the linkage between the project and the problem.
- In addition to the summary, your brief proposal must include:
- - A proposal narrative

- - A proposed descriptive project title.

- - The project start date (please choose a date at least three months in the future).

- - The number of months anticipated to complete the project.

- - An estimated amount of funding requested.

- - The project director's name, organization, title, address, phone number, fax number and e-mail address.

- In addition, your brief proposal must answer the following questions:
- - Has your organization received previous support from The Elder LifeCare Foundation?

- - Is this a request for continuation of a project previously supported by the Foundation? (If yes, please indicate the Grant ID number.)

- - Have you previously discussed your proposed project with an Elder LifeCare Foundation staff member? (If yes, please include his or her name.)

- - Which primary area best describes your proposal? (You may discuss primary and secondary areas in the paragraphs that address the points above.)
ELF primary areas: University Research, Care Management, Community and Family Healthcare, Insurance, End-of-Life Care, Behavior or Performance Management, Religious Organization, Long-Term Care Facility.

When your brief proposal is received at the Foundation, it will be reviewed promptly by the Office of Proposal Management to ensure the project described fits our goals and guidelines.
If your brief proposal does not fit our goals and guidelines, we will advise you immediately.
If your brief proposal fits our goals and guidelines, the project will be assigned to a program staff member for follow-up.
